MEWP Tip over - Whilst operating a MEWP with the platform raised at 60ft the MEWP tipped over whilst the operator was in the platform. The operator is in a life-threatening position.
Your investigation and understanding on why the MEWP tipped over when the platform was raised. Your investigation must cover all areas of concern understanding why the MEWP tipped causing the operator to become injured.
